Truth or Consequences: Advance Care Planning Essentials. April is Advance Care Planning Month in Virginia and this is our second annual community conference. The As You Wish Advance Care Planning Partnership invites you to join health care professionals and patient advocates in a healthy conversation, including a short documentary screening, on advance care planning do’s and don’ts.

Opening remarks via video by Senator Mark R, Warner. Keynote address by Karen Jones, broadcast journalist and author of fiction and non-fiction books including Death for Beginners, a practical guide for planning a graceful exit.

Advance Care Planning Across the Continuum is a program specific to health care organizations receiving notification of this program by email.

**Being Mortal program –  Based on the best-selling book by Atul Gawande, MD, the program includes a screening of the PBS FRONTLINE film Being Mortal  followed by informative discussion on what matters most to individuals and families and how to be prepared ahead of a medical crisis.
